Schools have immunization notification requirements stipulated in DC law and regulation (DC Official Code § 38–504 and DCMR 5-E § 5300.5).

The center must first put the letter on its letterhead and have it signed by the Authorized Representative. This letter must be distributed to participants (or their guardians) along with the Enrollment Form / Income Eligibility Statement (IES), and it should be given to them to keep.
