The documents below will assist you in planning and executing the administration of this year’s Health and Physical Education Assessment.

1. Sexual health exemption letter – This letter must be sent home to the parents/guardians of all students completing the assessment (grade 5, 8, and high school) prior to the administration date. The attachment includes English, Amharic, French, Korean, Spanish, Traditional Chinese, and Vietnamese translations. Prior to distributing to schools, LEAs can modify the administration dates listed in the letter (highlighted in yellow) as needed to reflect the LEA’s or school’s own administrative timeline, as long as the dates are within OSSE’s administration window of April 2 – June 15.
2. Upload templates – High school health class roster template and accommodations template. You may use these templates to transfer needed student information to OSSE via Box by March 15, 2018. More information on Box is coming Feb. 2018.
