Institutions must use the Claim for Reimbursement form for submitting monthly claims.
This letter should be given to parents or guardians of children enrolled in Tier II family day care homes whose providers have elected to distribute Income Eligibility Statements. The letter should be given to the parents or guardians to keep.
Must be completed at least once during the first four weeks of FSMP operations and must include the observation of a meal service. Sponsor must complete review even if the FSMP will operate for less than four weeks.
