Specialized Education State Complaint Form

Individuals or organizations may file a written, signed complaint alleging a violation of special education requirements under Part B or Part C of the Individuals with Disabilities Education Act (IDEA). The State Complaint Form is a model form that was developed to assist you in filing a state complaint. You are not required to use the form to request an investigation; however, all the information listed in the form must be included in your complaint.

For a complete description of the state complaint procedures, see 34 C.F.R. §§ 300.151-300.153 for IDEA Part B and 34 C.F.R. §§ 303.432-303.434 for IDEA Part C.
