The purpose of this form is to verify that an applicant is currently admitted into a state-approved educator credentialing program at your institution. Upon completion of this form by the certification officer at the educator program provider, this form is to be returned to the applicant to be submitted along with other required documents needed to apply for a DC educator credential.  If you admitted into an approved program with a DC-based educator program provider, please contact the certification officer at your institution to obtain your program admission verification form.
