How to Obtain a FBI Identity History Summary Check (IHSC)

All applicants applying for an educator credential issued by the District of Columbia’s Office of the State Superintendent of Education (OSSE) are required to submit a Identity History Summary Check (IHSC) report when filing their application.

To obtain a copy of your IHSC report, you must submit a request directly to the FBI’s Criminal Justice Information Services (CJIS) Division  or submit a request through an FBI approved “channeler” agency that has the ability to provide any United States citizen or legal permanent resident the right to request a copy of their own personal FBI criminal background check. IHSC reports must issued within the previous 12 months from the date an application is submitted for credentialing.

DC Public Schools (DCPS) Employees: Applicants currently employed and recently fingerprinted by DC Public Schools (DCPS) will not need to submit an IHSC report, unless the employee has a reported criminal incident or act in their criminal background report. In these instances, applicants shall be required to present an FBI IHSC and all related court documents to OSSE at the time of application submission detailing the incident and final status of the matter. Otherwise, current DCPS employees with no criminal incidents reported in their background will just need to confirm with the DCPS fingerprinting department that their clearance status is available so that OSSE can confirm the results electronically.

A. Request an IHSC through an FBI-approved channeler agency

Below is a list of FBI approved channeler agencies that receive fingerprint submissions and other relevant data and forwards that information to the FBI’s CJIS Division to receive IHSC results. Once completed the channeler agency disseminates the report back to you as the requester. To use a channeler, you must be a US citizen or legal permanent US resident. In most cases, when using a channeler agency you may expect to receive your results in just a few days.

To obtain an IHSC, you must:

  1. Select an FBI approved channeler agency from the table below.
  2. Find the “FBI Channeling” information section on that agency’s web site.
  3. Complete any agency registration process that may be required.
  4. Indicate that you are requesting an IHSC report for ‘PERSONAL REVIEW.’ A report for your personal review does not require an agency number or code and will be returned to you as the requester.
  5. Satisfy any additional requirements of the channeler agency to properly your obtain your IHSC.
  6. Receive your results back in the time as specified by the channeler agency.
  7. Once you receive your results, you must upload ALL pages of that report when submitting your online application.

B. Requesting your report directly from the FBI

If you wish to submit an IHSC request directly to the FBI as an individual, or if you are not a US citizen or legal permanent US resident, or you are outside of the US, you must follow the steps below. Making a request using this method will take approximately 12 to 14 weeks to process.

  1. Complete the FBI’s Applicant Information Form.
  2. Obtain a set of your personal fingerprints. Your fingerprints should be placed on a standard fingerprint form (FD-1164) commonly used for applicant or law enforcement purposes. The FBI will accept FD-258 fingerprint cards on standard white paper stock.
  3. Submit payment by credit card or by money order or cashier’s check in the amount of $18.00.
  4. Make sure to provide/submit all required information to ensure that your request is properly processed.
  5. Mail your request and other required documents to:

  6. Once you receive your results, you must upload ALL pages of that report when submitting your online application.
