DC Mayor’s Scholars Undergraduate Program Application

Please prepare all documents before you begin your application.

 

Type of Applicant

Required Documents and Information

Phase 1 Due December 1, 2015 Graduate of DC public high school (including DCPS and public charters) Graduation years 2005-2015

1. If currently enrolled in college: college transcript and GPA.

2. Proof of DC domicile/residency (unless you have already completed your 2015-2016 DC TAG application).

3. Student Aid Report (SAR)* (unless you have already completed your 2015-2016 DC TAG application).

4. 2015-2016 financial aid award letter* from eligible institution.

5. Other scholarship information, if applicable.

*If applicant is not FAFSA eligible and therefore does not have a SAR or financial aid award letter, they have the option of proving income eligibility in one of two ways:

1) Submit Federal Tax return

2) Prove SNAP or TANF eligibility (OSSE can verify. Documents are not required). They can provide financial aid award information via a copy of spring 2016 college invoice.

Phase 1 Due December 15, 2015 Older youth or adult learner who has passed the GED or earned a diploma through the NEDP in years 2005-2015.

1. GED or NEDP verification & scores.

2. Accuplacer, CASAS, or TABE scores* (unless student passed the GED with 157 or higher on all sections of the 2014 GED.)

3. Proof of DC domicile/residency (unless already submitted through DC TAG application).

4. Student Aid Report (SAR)* (unless already submitted through DC TAG application).

5. 2015-2016 financial aid award letter* from eligible institution.

6. Other scholarship information (if applicable).

*Scores cannot be more that 12 months old.

**If applicant is not FAFSA eligible and therefore does not have a SAR or financial aid award letter, they have the option of proving income eligibility in one of 2 ways:

1) Submit Federal Tax return

2) Prove SNAP or TANF eligibility (OSSE can verify. Documents are not required). They can provide financial aid award information via a copy of spring 2016 college invoice.

Phase 2 Due December15, 2015 All Applicants If applicant is given an award, s/he must accept the award and submit a Family Educational Rights and Provacy Act (FERPA) waiver.
