Successful transition planning requires coordination and collaboration to support students with disabilities in achieving their postsecondary education, employment, and independent living goals. This webinar defines the concept of interagency collaboration and describes several models that are currently being implemented in the District of Columbia (including the DC Secondary Transition Community of Practice and CIRCLES) to facilitate stronger relationships between families, schools, and various adult service agencies and improve transition outcomes for DC young adults with disabilities.
