According to the DC State Test Security Guidelines (page 2), "every school is responsible for returning all test materials to the test publishing company following procedures outlined in the Test Chairpersons Manual (pages 24-26)". Following the proper procedures for checking in and out test materials daily (by serial number and Test Administrator). Immediately documenting missing or destroyed materials on a Test Security Violation Form and sending that form to the LEA and OSSE. Failure to collect all testing materials, return all test materials as soon as the testing sessions have terminated or document the loss or destruction of test materials is a Test Security Violation per the DC State Test Security Guidelines and as allowed for in the Public Education Reform Act (PERA) Sections 38-2601 and 39-2602(b)(12).
