How to Submit a Complaint About Child Care Services

Any individual may submit a complaint about a child development facility to the Office of the State Superintendent of Education (OSSE). To submit a complaint, send an email to [email protected] or call (202) 727-2993. Complaints may also be faxed to (202) 727-7295. This contact information can be used to submit complaints for any matter related to a child development facility, including complaints related to child development facility compliance with licensing regulations; child, staff, or facility safety concerns; or the DC Child Care Subsidy Program application and/or eligibility determination.

How to Submit an Unusual Incident Report

Child development facilities are required to report unusual incidents to OSSE. An unusual incident is any accident, injury, or other extraordinary event that involves a child in care, a staff member, or the operation of a child development facility, including suspected child maltreatment or abuse.

Child development facilities must use the Unusual Incident Report (UIR) form to report unusual incidents. UIR forms should be submitted to OSSE at [email protected].
