Enrolled participants in adult care, child care, and family day care homes must complete and submit a completed Enrollment Form/Income Eligibility Statements upon enrollment, and annually thereafter.

This form should be completed by subsidized child care providers for  monitoring compliance with the Provider Agreement for Subsidized Child Care Services, Section 1 (A) (2) – Ages and Stages Questionnaire (ASQ).

These documents should be used when soliciting quotes for a food service contract with an estimated value of less than $100,000 for one year of services.
