Each school/campus site is required to submit to OSSE a 2015 School Test Plan, due ten days before the administration of a Districtwide assessment. To help schools develop their own plan, OSSE has released a 2015 Sample Plan Template. Each segment of this template is aligned to the 2015 Test Plan Rubric and closely follows the 2015 General Observation Form used by State OSSE Auditors for observation. Sample answers for each section are identified in “red” font and highlighted in green.
