Report Noncompliance with COVID-19 School Health and Safety Requirements

Mayor’s Order 2021-097, issued on July 29, 2021, authorizes the Office of the State Superintendent of Education (OSSE) to take enforcement action with respect to DC Health’s mandatory health and safety guidance provisions. These enforcement actions apply to public, public charter, private, parochial, and independent schools.

Individuals who wish to notify OSSE that a pre-K-Grade 12 or adult education school may not be adhering to a requirement from the DC Health Guidance for Schools may do so by providing all the required information through this online form.

OSSE will acknowledge receipt of the notification and share the information provided with the school. Schools are expected to acknowledge receipt of the information, review the information, and, if needed, take any follow-up corrective action.
