DCLEAP Required Supporting Documents
The following documents must be mailed or hand delivered, at the same time, to Higher Education Financial Services to complete the application process for DCTAG.
Student Aid Report (SAR), PDF or eight (8) page paper version, as provided by the US Department of Education after the completion of the Free Application for Federal Student Aid (FAFSA).
Certified DC Income Tax Return (D-40), including Schedule "S" with the applicant listed as dependent (if applicable), for the most recent calendar year. To learn how to verify certification of your D-40 electronically please see D-40 Electronic Certification.
Twelve (12) Month Income/Benefit History Statement (for the previous year) from either of the following sources: Temporary Assistance for Needy Families (TANF), Social Security Income (SSI), Social Security Disability Income (SSDI), Retirement Income, Child Support, Worker's Compensation, Veterans Administration, or Unemployment.
One Current Utility Bill: phone bill (land line, not cell phone), bank statement, pay stub or mortgage statement, no older than forty-five (45) days from the date of the DC OneApp submission that reflects the name and address of either the applicant or their parent/legal guardian.
Proof of High School Completion. New applicants only must provide proof of high school completion (i.e. high school diploma, General Equivalency Diploma (GED) certificate, college acceptance letter or college transcript).
Unofficial College Transcript. Renewal applicants only must submit an unofficial college transcript from the last semester completed.
Affirmation and Legal Disclaimer Statement with original signature from the applicant and the parent/legal guardian (if applicant is a dependent). The statement is available at the completion of the DC OneApp online application.
All required supporting documents must be mailed or hand delivered, to HEFS at the same time to be considered for an award.
Applicants who are wards of the DC Court must provide:
- Ward of the Court verification letter
- Proof of high school completion
- One Student Aid Report (SAR)
- Affirmation and Legal Disclaimer Statement with Applicant’s original signature
